General Office Policies

Our office has the flexibility of appointments or walk-in care.  Please bring your insurance card for all visits.

 

New patients are encouraged to download and print out all forms before their first visit.

 

Prescription Refills

We have a 24 hour message line that you can call to request a refill on prescriptions.  The phone number is: (239) 333-1768.  Please be sure to leave your name, date of birth, your telephone number, the pharmacy name and phone number you wish us to call.  Also include the name and dose of the medication you need.  Please understand that this process may take at least one business day and some medications may require an office visit before they can be refilled.

Payment Policies

We accept most major insurances.  All Co-pays and other fees for our services must be paid at the time of your visit.

 

Cancellation Policy

If you must cancel your appointment please call us as soon as possible.  Failure to call to cancel an appointment may result in a charge to you, which is not covered by insurance companies.
